Feel free to start using this document from now in (In this new thread only) however it will become compulsory to submit this form with every order of services at Bootle from this coming Monday.

That form is rather bright to say the least! Perhaps tone it down a bit and remove the checkerboard effect?

That last one didnt contain any data. How about this one which I edited from the orginal. I have also included some forumlas, so the cost of each service is automatically calculated using the cost collum and and no of vehicles collum, also the total cost at the bottom right is automatically done too.

il use those ideas and put them into making a new catalogue then. it will be in the layout that iv published, purely because its easier to state what vehicles may be submitted, and their types, and any notes specific to certain vehicles that need to be made.
